• Monday-Friday: 8 Hours

1. 3/3/25-3/7/25
2. 4/28/25-5/2/25
3. 6/30/25-7/4/25
4. 8/4/25-8/8/25
5. 9/15/25-9/19/25
6. 11/10/25-11/24/25

SUBMISSION REQUIREMENTS : 

• Board Certified Only: REQUIRED
• Active WA License or IMLC: REQUIRED
• Certifications BLS: REQUIRED
• Availability for the above dates: REQUIRED

POSITION DESCRIPTION : 

• 100% 18 + population
• Patients per day: 15
• Percent of Practice: Office/Clinic: 99%, Hospital: 1%
• New Consults per week: 5
• Number of Oncologist in Practice: 1 RadOnc, 2 MedOnc,
• Number of other healthcare professionals in practice: 1 Dosimetrists, 1 Physicist, 4 Radiation Therapists
• Is locum tenen provider required to supervise APP's? No
• X-Ray Onsite
• RN's, MA's RN Navigators, Social Worker, Occupational Therapist
• Treatment Plan Computer System: Eclipse
• Office Equipment: Varian IX, Varian TruBeam, HDR Machine, GE Wide Boar CT Scanner
• EMR: Epic
