• Practice Setting: Office - Clinic  with procedures

• Documentation System/EMR: EPIC
• Trauma Level/Designation: Level 2
• Annual ER Volume: 60,000
• Number of Operating Rooms: 12   
• Description of Ancillary Staff: MA 
• Hemodialysis Access? Yes
• Patient Age Demographic: Adult / Geriatric 
• REQUIRED Procedures/Skills:
• Vascular Surgery Procedures Required: will determine with the provider
• Endovascular Surgery Procedures Required: will determine with the provider

SUBMISSION REQUIREMENTS : 

• Board Certification - REQUIRED
• Active OH license & DEA - REQUIRED
• ACLS/BLS - REQUIRED
