Practice Setting: Outpatient

Dates of Coverage: 9/29/25, 9/30/25, 10/1/25

Schedule (Shifts/Hours): 7am - 4pm

Number of Physicians in Practice: 1

Will There be Other Radiation Oncologists Present During Locum Tenens Coverage: No

How Many APPs Provide Coverage: N/A

Average Number of Patients Per Day - OUTPATIENT (Clinic/Office): 35 treatment patients

Patient Population for Required Coverage: Adult, Office/Clinic

Documentation System/EMR: Mosaiq, Epic

Required Procedures:

Conventional Radiotherapy, 3D Radiotherapy, IMRT

External Beam Radiation - 3D-CRT, External Beam Radiation - IGRT, External Beam Radiation - IMRT

Photon Beam Radiation Therapy, Stereotactic Radiosurgery, Stereotactic Body Radiation Therapy (SBRT), Fractionated Stereotactic Radiotherapy, Orders Images, Prescribes, Orders Tests

~Estimated Credentialing Timeframe:  90 days

Submission Requirements

• Board Certified - REQUIRED
• BLS - REQUIRED
• Active OH license - REQUIRED; willing to accept IMLC candidates
• Active OH DEA - REQUIRED
