License: If provider has an IMLC it must be listed as a license.

Other Details: Give providers availability for the shifts below. We will prioritize providers who are able to cover all/most of the shifts.

Day shifts are 11.5 hour shifts.

Admit shifts (Admit/CDU, PM Admit, and Night) are 10 hours, 10 hours, and 12.5 hours, respectively. Admit/CDU and night shift hours are subject to change slightly, as we may shorten night shift and lengthen Admit/CDU.

Cross cover (APP Noc) is 10 hours.
