Schedule/Availability Requirements:

Start Date: ASAP for 6-12 months

Schedule/Shift times: Fulltime M-F 8a-5p every week

Setting: Outpatient

Duties:  Hematology Care - It's Hem/Onc but we really need coverage in the benign and malignant heme. Just no solid tumor. The benign versus malignant ratio is probably 60/40, as an estimate. 

EMR: Epic

Support Staff: MA, RN and Scheduler

Reason for Coverage: Vacancy

Dealbreakers: Availability 


Credentialing Information:

  • Timeframe: 60-90 days

  • Credentialing with other facilities?

Submission Requirements

  • Active Board Certification – Board Certified Hematology [Required]
  • State License – Will consider IMLC LOQ. – [Required]
  • Residency Requirements/Fellowship – [Required]


Active Other Certifications 

  • ACLS - [Nice to Have]
  • BLS - [Required]
  • DEA [Highly Preferred] - must have by start date.
  • Clean Background/Malpractice –  [Required]
  • The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
