Description : Facility is looking for a compassionate Hematology/Oncology locum to provide high quality, patient centered care for many types of cancer and blood disorders for a community based ambulatory clinic in Burin WA. Details below. 

Setting: Community Outpatient Clinic 

Duties: Managing exisiting hematology and oncology patients for a physician who is 

Required Procedures: No procedure. 

EMR: EPIC, Beacon

Patients per day: 12-15

Support Staff: RN and an MA. 

Credentialing Information:

Timeframe: Average ~ 3 months

 Submission Requirements

• Board Certification – BC [required] 
• State License –WA license or IMLC [highly preferred]
• Other Certifications – BLS [required]
• Malpractice – Clean [highly preferred] 
• Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
