Requirements & Qualifications

• Certification: Board Certified (BC)
• License: Washington (WA) Required
• Credentialing: 90 Days
• EMR: Epic

Required Surgical Procedures

The provider must be proficient and experienced in the following:

• Aortic Valve Conservation: Required
• Aortic Root & Remodeling + Enlargement: Required
• Impella 5.5 (Implant & Support): Required
• Minimally Invasive Mitral Surgery: Preferred
