• Board Certified in Neonatology (not accepting BE) - [REQUIRED]
• TX license  [REQUIRED] no exceptions for quick credentialing
• Cannot accept providers who have not been clinically active for 1 year [REQUIRED]
• Providers must have at least 6 months of clinical experience within the specialty of job they’re applying for [REQUIRED]
• Must have completed Prescreen Questionnaire (see attachment for form) [REQUIRED]
• CV must be in MM/YYYY format with all gaps explained [REQUIRED]
