EMR: EPIC Coverage Dates: As soon as credentialed - June 2027 Shift Schedule Details & Shift Duration: 24 hours, 6:30am - 7am next day Is call required for this assignment? No additional call required

of ORs : 289 beds , 12 ORs

Type of Anes Model: Supervision Supervision ratio: TBD Case types: All, no cardiac or neuro. Most likely heavier OB than general/ortho **Is there a specific weekend day preferred (SAT vs SUN)? Flexible

REQUIREMENTS:

• Minimum # of shifts per month: 4 shifts per month
• Life Certifications/Qualifications required: ACLS + BLS + PALS
• Licensure : PA license preferred; will look at a provider with multi-state license or full licensure candidate
• Credentialing Timeframe: approx. 60 days (file must go to 2 committee meetings before privileges are granted) Temps Available? (Requirements for temps, timeframe): No
