• Average Number of Patients Per Day - OUTPATIENT (Clinic/Office): 35 treatment patients
• Patient Population for Required Coverage: Adult, Office/Clinic
• Documentation System/EMR: Mosaiq, Epic

REQUIRED PROCEDURES : 

• Conventional Radiotherapy, 3D Radiotherapy, IMRT
• External Beam Radiation - 3D-CRT, External Beam Radiation - IGRT, External Beam Radiation - IMRT
• Photon Beam Radiation Therapy, Stereotactic Radiosurgery, Stereotactic Body Radiation Therapy (SBRT), Fractionated Stereotactic Radiotherapy, Orders Images, Prescribes, Orders Tests

SUBMISSION REQUIREMENTS : 

• Board Certified - REQUIRED
• BLS - REQUIRED
• Active OH license - REQUIRED; willing to accept IMLC candidates
• Active OH DEA - REQUIRED
