- Average Number of Surgical Cases Per Day: 1-2 emergent
- Trauma Level/Designation: 1
- Documentation System/EMR: EPIC
- Number of Neurosurgeons in Practice: 1 employed 1 non-employed
- Patient Age Demographic: (Adolescent, Peds, Neonate, Adult, Geriatric)
- Adult and Geriatric but emergent cases through ED
Procedures Required:
- Neurosurgery Cases ADULT
- Neurosurgery Cases TRAUMA
- Brain Bleed Stoppage
- Clot Removal
- Tumor Removal
- Trauma Repair
- Shunt Insertion or Revision
- have neuro IR in house
Spine Surgery Procedures Required:
- Spine Stabilization and Reconstruction
- Vertebroplasty and Kyphoplasty
- Spinal Fusion
- Laminectomy
- Diskectomy
Submission Requirements
- Board Certified Neurosurgery - [REQUIRED]
- OH License no exceptions - [REQUIRED]
- Provider must be available for March dates to be considered no exceptions - [REQUIRED]
- A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
- BLS, ACLS - [REQUIRED]
