• June 01 - July 31.  Daily coverage; Monday – Friday; 6:00a – 2:00p
• Setting: Surgery Center
• FTE: 1.0
• Duties: Provide anesthesia

• Malpractice – Clean Preferred [HIGHLY PREFERRED] 
• Availability at time of name clear – Required [REQUIRED]
• Self-Query NPDB report is required at the time of presentation (Should be pulled within 30 days) - [REQUIRED]
