We need a provider who can cover the below dates in 2026. ONLY submit who can cover these exact dates.
- Dec 14 – 18
- Dec 21 – 28 (weekend call through 7am 12/28)
- Dec 28 – 31 (no New Years Day)
- Ongoing in 2027
Schedule 7am to 3pm. 8 hr shift. This cannot be increased to 10 hr shift.
The ideal candidate will be:
- BC
- IN License, CSR, DEA
- Drive in candidate
- Can commit to all dates
- Sign Anesthesia Attestation for all procedures
- Fully Credentialed with their locum agency
- Commit to orientation Friday, Dec 11, 2026
- Will need case logs
Candidate Requirements
- Active IN State License or IMLC.
- Board Status: Board Certified or Board Eligible (Certification must be obtained within 5 years of residency).
- Minimum Years Experience (if applicable): None Required
- Flu vaccination: Required or Need Medical or religious exemption.
- Covid vaccination: Required or Need Medical or religious exemption.
- Certifications: ACLS - Required if requesting conscious sedation privileges, BLS – Required for all specialties.
