• Coverage Type,Clinic Only (No Call) 
• Schedule,"Monday – Friday, 8:00 AM – 5:00 PM" 
• Volume,Minimum 18 PPD (Patients Per Day) 
• EMR,Epic (Chart closure required within 72 hours)

Candidate Requirements

• Experience: Must have prior FQHC experience.
• Clinical Skills: Comfortable treating Behavioral Health (Anxiety) and managing Pediatric Chronic Care.
• Board Status: Board Certified (BC).
• Certifications: BLS & Active DEA (required before start).
• CV Standards: Must include Month & Year for all work history; all employment gaps must be explained.

Credentialing & Compliance

• Timeline: 30–60 Days (Decision usually made within 7–14 days of submittal).
• Mandatory Immunizations (Must be cleared before start):
• Flu Vaccine
• TB Status: Results/X-ray within 90 days of start date.
• Titers (Positive) or Full Series: MMR, Hep B, and Varicella Zoster.
• Tdap: Must be within the last 10 years.
