LOCUM- Neurosurgery *PLEASE NOTE: Provider MUST be Board Certified and have a CLEAN FILE to be considered as we can not get privileging done by 4/30 without one. FLURO REQUIRED* JOB DETAILS: Guaranteed hours: 8 hours per day OT to apply after 9 hours per day We are looking for 1-2 providers who can cover the dates below. Provider will be expected to cover both clinic and call coverage during these stints. Provider will be expected to be in clinic and on call from 830am-5pm on weekdays. They will then be on night call from 5pm-8:30am the next day. Weekends will be 24hr call coverage. We would like to note they may not be in clinic all 5 days during the dates listed but it is a possibility. April 30 8:30a- May 5 8:30a (can start 5/1 8:30a also but prefer 4/30 start) June 1 8:30a-June 8 8:30a More dates to be offered. Will see complex spine cases No airway certifications required REQUIREMENTS: FLURO certification is required (they can have the temp FLURO) Active CA License and CA DEA Must be Board Certified or truly Board Eligible within 3-5 years of residency is an option but we are looking at BC.

PRIVILEGING: We will need providers to submit during the privileging process 100 cases within the past 24 months specific to the privileges being requested. Please note that the privileging timeline will not begin until the RFA and application are turned back in. - Privileging Timeline: 90 days (average) - RFA due within 48-72 hours (request for application); - Hospital Privilege Application due within 5 business days - If temporary privilege is applicable, eligibility can only be confirmed after completed file (application and all verifications received) are sent to MSO - Deviation from this timeline may result in the provider being cancelled from the site. - o All specialties require 2 years of current Hospital clinical activity in the specialty. o Gaps greater than 3 months must be explained. o Include current number of case logs upon presentation for surgical specialties.
