Looking for 1-3 providers depending on availability 

Must be BC or BE within 5 years. 

ABEM/ABEOM certified (Will not accept ABIM/ABFM)

Average number of patients in 24hrs is 110. 

o All HBS specialties require 2 years of current Hospital clinical activity in the specialty.

o Gaps greater than 3 months must be explained.

o Include current number of case logs upon presentation for surgical specialties.  

Beds in Hospital: 32

Minimum Board Certification : BC 

EMR System:Epic

Board certification required :  ACLS / BLS

License requirement : Licensed in CA only

Estimated Credentialing Time :90-120 days
