JOB DETAILS:

We are in need of 1-2 providers to help with covering a need at the facility. Schedule will be Monday- Friday from 7am-4pm. Ideally providers should be able to cover one month on, one month off or more than 2 weeks on, 2 weeks off.

No call in this position.

Please ensure bids reflect the below:

- Guaranteed Hours: 8

- OT to apply after 9 hours

- OT should be preapproved by the medical director via email or text.

The work will be less critical cases.

REQUIRED:

- Active CA License and CA DEA

- Board Certified or truly Board Eligible (within 5 years of residency)

PLEASE ENSURE AN UPDATED CV WITH ALL HOSPITAL AFFILIATIONS IS UPLOADED DURING PRESENTATION.

Attached is a new privileging form that will set expectations for what is needed once a provider is confirmed. Please consider starting to ensure your provider has all of these items to ensure they can qualify for privileges at the time of confirmation.

o All HBS specialties require 2 years of current Hospital clinical activity in the specialty.

o Gaps greater than 3 months must be explained.

o Include current number of case logs upon presentation for surgical specialties.
