Description : 

• Type of privileges (clinic/hospital/both): Both
• Setting:  Inpatient, Outpatient or Both? Outpatient
• Rounding, admissions, or none? None
• Average patients per day: 12-16
• Required clinical activity in the last 24 months (case logs): Applicant must provide documentation of provision of HEME ONC services representative of the scope and complexity of the privileges requested during the previous 24 months.
• Will provider work solo? If not, other providers specialties working with them: Yes
• EMR: EPIC. Proficient User. 
• Equipment - EPIC Charting
• -5 days/month in person clinic, 1 hr telehealth conference every Thursday, ad hoc telehealth clinic as needed, and ability to intermittently answer staff messages
• -8 hour shift ( Standard 5, Day (8 Hours) 7:00 - 15:00 )
