Modalities: XR, CT, MR, US, NM

Subspecialty Case Mix: (40) ER, (60) Neuro, (5) Nuc Med

Daily Volume Expectations (per 8 hr shift)

Volume: 90-100

wRVUs: 60-70

Submission Requirements

Board Certification - [REQUIRED]
Active WV License - [REQUIRED]
Must have Neuro Fellowship - [REQUIRED]
DEA Registration - [REQUIRED]
A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
