Documentation System/EMR: EPIC

??Submission Requirements

Board Certified - Hematology/Oncology - REQUIRED
Active VA License - REQUIRED
BLS - REQUIRED
Clean background - highly preferred
Provider must be local - only mileage is reimbursed
