Primary Care Locum Opportunity in Oregon

Hospital is looking for Primary Care locum coverage for 9-12 months. Details below!

Schedule/Availability Requirements:

Assignment: T-F and needs to be able to provide consistent coverage for up to 12 months

• Start date: As soon as credentialed 
• Shift times: Weekdays: 7:30am-5:30pm 
• Frequency: Tuesday-Friday - Full time coverage 

Setting: Primary Care

FTE: 1.0

# Providers Needed: 1

Duties: Primary care provider covering ages newborn to geriatric care. - we are looking for a provider to service mainly 18 years and above but may be asked to occasionally see teenagers. 

Required Procedures: primary care procedures; circumcisions potentially 

EMR:  eClinicalworks

Support Staff: 1 MA, receptionist; clinic RN as needed

Reason for Coverage: Have not been able to fill full time need 

Deal Breakers? not being available T-F and needs to be able to provide consistent coverage 

Additional Questions:

• Admissions per day, Patients per day:  17-20 patients per day
• Will you consider Internal Medicine or Family Medicine Physicians? No Primary Care  

Credentialing Information:

Timeframe: Temps can be issued 

Billing enrollment: Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less?  No

Submission Requirements

• Board Certification –  BC in Family Medicine [Highly preferred] will accept truly BE
• State License –  OR [Required]
• Other Certifications –  BLS[Highly Preferred] can be obtained during the credentialing process
• Malpractice – Clean [Highly preferred]
• A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
