Practice Setting: Inpatient and call

Schedule: Day 8 hours

Call Schedule: 24 hours on call

Call Ratio: 1:4

Average Number of Clinic Patients Per Day: 20

Average Number of Inpatient Admissions Per Physician Per Day: 11

Trauma Level/Designation: 1

Total Number of Beds: 400

Number of ICU Beds: 50

Number of Cardiologists Physician in Group: 20

Will Other Cardiologists be Present During our Locum's Coverage: Yes

Will the Locum Physician be Required to Supervise Other Health Care Providers: 1 APP

Patient Age Demographic: Adult Geriatric

Documentation System/EMR: EPic

Non-Invasive Procedures Required:

Electrical Cardioversions, Electrocardiograms (EKGs), Tilt Table Tests, All electrophysiology procedues

Estimated Credentialing Timeframe: 120 days

Facility reimbursement for the following costs: Mileage Billed at the IRS Standard Rate, Rental Car, Hotel, Flight

• A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
