Submission Requirements
- Board Certified Urology - Preferred. Will accept Board Eligible providers if they are within 5 years from residency
- Case logs will be requested for reviewal at presentation and offer.
- North Dakota License - Highly Preferred
- Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
