Description

We are seeking a Board Eligible Geriatrics Physician with strong experience in post-acute care to provide comprehensive medical oversight for a Skilled Nursing Facility (SNF) and Long-Term Care (LTC) unit. Located on a major medical center campus, this role manages a complex geriatric population to ensure regulatory compliance and continuity of care during a provider vacancy.


Clinical Schedule & Pacing

  • The Shift Structure: Part-time continuity model consisting of three 10-hour days per week (Facility preference: Tuesday, Wednesday, and Thursday).
  • Shift Times: 7:00 AM – 5:00 PM.
  • Call Profile: Absolutely No Call. All overnight and weekend call tracking is fully managed by the hospitalist team.
  • Advanced Practice Support: The incoming physician will have the benefit of a dedicated Advanced Practice Provider (APP) assisting with daily clinical operations.


Core Responsibilities

  • Post-Acute & Rehabilitation Care: Review and approve SNF admissions; complete comprehensive H&Ps, conduct required CMS compliance/recertification visits, and manage acute changes in condition to minimize unnecessary ER transfers.
  • Long-Term Care Oversight: Provide ongoing primary medical care for permanent residents, focusing on complex chronic disease management, frailty, advanced dementia, annual wellness assessments, and palliative/comfort-focused care plans.
  • Interdisciplinary Leadership: Collaborate with nursing, therapy, case management, and consultant pharmacists to update care plans and participate in goals-of-care or family conferences.


?Requirements

  • Certification: Board Eligible (BE) in Internal Medicine or Family Medicine. Significant experience in Skilled Nursing, Long-Term Care, or Transitional Care is strongly preferred.
  • Licensure: Active New Jersey (NJ) Medical License or an active, assignment-ready IMLC passport.
  • Life Support: Active BLS and ACLS certifications.
