JOB DESCRIPTION : Open dates:
- 7-14 days a month ongoing
Job Details:
- Practice Setting: Hospital Based
- Schedule (Shifts and Hours): 24 Hour Call
- Number of Operating Rooms: 1 OR and 1 Siemens Hybrid OR and 1 Siemens Interventional radiology suite
- Hemodialysis Access on Site, In-house
- Patient Age Demographic: Adult, Geriatric
- Ancillary Staff: Vascular team is on-call Thursday-Sunday for the rest, no certified first assist but team is well trained and 2 surgical technologists are provided.

Vascular:
- Aneurysm repairs
- Abdominal
- Arterial bypass
- Peripheral
- Vascular Access Devices
- AV shunt
Endovascular:
- Bypass Surgery
- Femoral Popliteal Bypass
- Aortofemoral Bypass
- Stenting
- Endovascular and Open Surgical Reconstruction for Deep Vein Occlusions
- Peripheral Artery Disease
Estimated Credentialing Timeframe: 90 Days
Submission Requirements

- A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
