SCHEDULE: Schedule: Tues-Fri for 6 months Shift time: 0800 - 1630

Start Date: July 28, 2026 (onboarding July 27)

Schedule: Tues-Fri for 6 months

Shift time: 0800 - 1630

Setting: Ambulatory Clinic

Duties: Oversee treatments, New consults, follow ups, planning, status checks, tumor conference participation

Required Procedures: SBRT, IMRT, 3D, SRS

EMR: Epic

Support Staff: 1 RN, 4 Radiation Therapists, 2 medical physicists, and 3 Dosimetrists

Reason for Coverage: Provider resignation

Credentialing Information:

Timeframe: can emergency credential in a week once a completed application is returned

Requirements:

Credentialing with other facilities? No

Billing enrollment - Is this position providing services in lieu of an existing enrolled provider for a period of 60 days or less? No

Submission Requirements

Active Board Certification – Board Certified or Board Eligible [Required]

KY License or IMLC (LOQ in hand)  – [Required] 

Residency Requirements/Fellowship –[Required]

Active Other Certifications – 

• ACLS - [Nice to Have] 
• BLS - [Preferred] Must have by start date.
• PALS - [Nice to Have] 
• DEA - [Preferred] Must have for assignment but can do a prescription plan if not issued by start date
• NRP - [Nice to Have] 

Clean Background/Malpractice – [Highly Preferred] Can consider minor/closed cases on a case-by-case scenario with NPDB.
