Description : 

Georgetown is looking for a provider to care for patients while our community provider is out on leave. We need the specific dates on the coverage page The anticipated schedule is: Monday - Thursday 8 - 430 Active KY license will be needed due to the tight turnaround time We cannot consider IMLC due to the short turnaround time No treatment planning will be needed Need onsite provider. • Vendors are responsible for obtaining and submitting all necessary pre-hire requirements including a completed background check and drug screen to People Services as part of provider onboarding. • Medical Staff Services/CVO will conduct separate background screening and credentialing reviews required for privileging and credentialing •
