Hospital is seeking a Urologist to assist with clinic and call coverage!
Urgent dates of coverage needed (ongoing needs continued into 2026 TBD):
- Monday, December 1 at 7a – Friday, December 5 at 5p
- Monday, December 8 at 7a – Friday, December 19 at 5p
- Monday, December 22 at 7a – Friday, December 26 at 5p
- Clinic and Call coverage. Ideally, they would like a provider to cover clinic/hospital call which would include rounding, clinic patients, consults, etc.
- Adults only
- 2-5 ppd
- EMR: EPIC
- General Urology procedures required
- Willing to pay for full travel
Credentialing Timelines:
a. Payer enrollment: 90-120 days from receipt of Demographics, IN license, IN Controlled Substance Registration, DEA w/IN practice address, and signed enrollment forms.
b. Hospital Privileges: 45-60 days from receipt of complete CVO/CPI Portal Application.
Submission Requirements
- BC or BE within 5 years of residency - [REQUIRED]

- CDS Certification and DEA Registration - [REQUIRED]

- Robotics trained - [HIGHLY PREFERRED] - Please note whether the provider has this training at the time of name clear
