Availability Requirements

The facility has a strict preference for consistent, long-term coverage. They will not consider providers with limited or fragmented availability.

Weekly Commitment: Minimum 3–4 days per week in person.
Monthly Commitment: Minimum 3 weeks per month.
Duration: Must be able to commit to the majority of the October 2026 – July 2027 window.

Provider Qualifications

Licensure: Active Indiana (IN) Medical License.
Board Status: Board Certified or Board Eligible (BE).
