Hematology Oncology Locum to Perm Opportunity in Indiana

Health System is looking for an Outpatient long term Hematology/Oncology provider who has interest in going perm and helping build the program to start ASAP! 

• Practice Setting: Outpatient Hematology/Oncology  
• Coverage will include two locations
• Reason for Coverage: New service line 
• Dates of Coverage: ASAP - ongoing
• Patient Population for Required Coverage: Adult, Geriatric
• What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %?: Hematology 50%; Oncology 50% - 0% IM 
• Average Number of Patients Per Day - OUTPATIENT (Clinic/Office):  30 average
• Average Number of Patients Per Day - HOSPITAL (Call/Rounding): 0
• Clinical Area: Oncology - Outpatient Setting
• Documentation System/EMR: EPIC
• Estimated Credentialing Timeframe: With an IN license 60 days; without IN license 90 days

Required Procedures: 

• Mgmt. of VAD/CV Catheters
• Bone Marrow Biopsy

Required Duties: 

• Participation on a Tumor Board
• Mgmt. of Bone Marrow Transplants
• Orders Tests
• Blood Bank
• Orders Images
• Prescribes

Administration of Chemotherapy: 

• Physician
• Nurse oncologist

Submission Requirements

• Board Certification (provider must have AMBS/AOA board certification in one, and competency in the other demonstrated) - [REQUIRED]
• Active IN State License or IMLC with LOQ in hand - [REQUIRED]
• BLS - [REQUIRED]
• Provider must be willing to commit to long term assignment (Minimum of 6 months) - [REQUIRED]
• A Self-Query NPDB report is required at the time of presentation (Should be pulled within 30 days) - [REQUIRED]
