Monday-Friday, 8:00am-5:00pm

We have re-opened a Gastroenterology-Outpatient/General physician locum . 

PLEASE READ THE PRACTICE PROFILE FOR DETAILED UPDATES ON THE JOB DESCRIPTION, AVERAGE CASES, REQUIREMENTS, AND SCHEDULE.  

 BC or BE in Gastroenterology, ACLS, BLS, Moderate Sedation. 

LICENSE RESTRICTION: ALREADY ACTIVE Illinois license and active IL DEA and CSR are required. 

SCHEDULE: As soon as credentialed, payer enrollment completed, and completion and submission of Health Attestation Form.  Monday-Friday, 8a-5p, ongoing (Outpatient Clinic and GI Procedure Lab). Start time for MAC blocks is 7:30a. PLEASE DO NOT SEND CANDIDATES THAT CANNOT WORK AT LEAST ONE FULL WEEK PER MONTH!   

Submission Requirements

Board Certified or Board Eligible in Gastroenterology - Required

Active IL License, DEA, CSR – Required

Availability per request - Required

Stratum Med Cover Sheet in Word format

Two references per Stratum process – Required

Clean and easy-to-credential candidates – Highly Preferred
