• Number of Physicians: 8 (combination of total join, spine, hand, podiatry)
• Number of advanced Practice Providers: 8
• Supervision of APPs expected: No
• Nursing Support: nursing and APP support in OR
• Services offered in the practice: Any ortho traumas that come in (Trauma level III). 
• Medical Imaging: Yes
• Lab & Pharmacy: Yes
• Clinic Provider Daily Volume: N/A
• Average Daily Hospital Practice Census: 230-250

Submission Requirements

• Board Certified in Orthopaedic Surgery - [REQUIRED]
• Active IA and/or IL License or Active IMLC (please upload LOQ at time of name clear) - [REQUIRED]
• Fellowship Trained in Orthopedics - [REQUIRED]
• Must be experienced - [REQUIRED]
