Provider Qualifications

• Licensure: Active Florida (FL) Medical License.
• Board Status: Board Certified (BC) Only.
• Mobility: Willingness to float between various local facilities is mandatory.

Credentialing & Timeline

• Standard Window: 90 Days.
• Urgency: Critical. With only 26 days until the May 4 start date, the facility is well inside its standard 90-day window.
• Submission Strategy: Only submit providers with a clean Florida file and MQSA credentials in hand. You will likely need to request a "Temp-to-Perm" credentialing path to hit the start date.
