Description : 

Hospital is seeking a locum Hematology/Oncology provider to start as soon as credentialed. 

Practice Setting: 4 days outpatient and 1:3 cover  

Reason for Coverage: 3 month maternity leave

Dates of Coverage: Sept-Dec 2026

Patient Population for Required Coverage:  Adult, Geriatric, OB

What Percentage of the Practice is Hematology %, Oncology %, Internal Medicine %?:  Hem -60%

Number of Physicians in Practice:  3 --one having baby

Will There be Radiation Oncologists Present During Locum Tenens Coverage? yes

How Many APPs Provide Coverage:  2 

Average Number of Patients Per Day - OUTPATIENT (Clinic/Office): 15-16

Average Number of Patients Per Day - HOSPITAL (Call/Rounding): Approx 1-5 

Clinical Area:  Oncology - Outpatient Setting, Hematology - Outpatient Setting, Oncology - Inpatient Setting, Hematology - Inpatient Setting

Trauma Level/Designation (Hospital):  2 level 

Documentation System/EMR:  EPIC

Administration of Chemotherapy:  Nurse oncologist

Estimated Credentialing Timeframe:  TBD-we have approval for expedited start if necessary

Provider Must be local - only mileage is reimbursed

Required Duties: 

• Participation on a Tumor Board
• Orders Tests
• Orders Images
• Prescribes

Submission Requirements

• Board Certified -  [REQUIRED] 
• CT license in hand at the time of submission - NO exceptions - [REQUIRED] 
• BLS - [REQUIRED]
• Provider Must be local - only mileage is reimbursed
• A Self-Query NPDB report is required at the time of presentation and must be pulled within the same month as the submission. - [REQUIRED]
