- Board Eligible (within 5 years of residency) or Board Certified in Urology - [REQUIRED]
- Active CO License (or willingness to obtain) - (note in "Additional Submission Details" field)- [REQUIRED]
- BLS and ACLS - [REQUIRED]
- Most recent NPDB Self Query Report (within 30 days active) - [REQUIRED] * no exemption, required at presentation
- Visa Support: J1 or H1B candidates are eligible for visa sponsorship. (note in "Additional Submission Details" field)- [REQUIRED]
- DEA (must be obtained prior to start) - (note in "Additional Submission Details" field) - [REQUIRED] * no exemptions
- Document care using the Epic EMR system
