**PREFER ONE PROVIDER THAT CAN WORK BOTH SETS OF DATES, REQUIRED TO HAVE A CLEAN FILE AS NOVEMBER DATES WILL REQUIRE TEMPS** If a provider does not have a clean file they will not be pushed forward for review.

We don't do central line placement outside the newborn period (UVC and UAC are technically central lines but only for neonates)
chest tubes, intubations for pediatric patients would most likely occur in the ED. ED docs and anesthesia would typically have significantly more experience here.

Please note that the privileging timeline will not begin until the RFA and application are turned back in. Typical turn around for RFA is 48-72 hours.
o All specialties require 2 years of current Hospital clinical activity in the specialty.
o Gaps greater than 3 months must be explained.
o Include current number of case logs upon presentation for surgical specialties.
Provider must have an active CA license and they must be BC or truly BE within 3-5 years of residency.
