• Feb 13-15 (ending 16th am)
• EMR: Cerner
• Support Staff:  OR

Submission Requirements

• Board Certified or Board Eligible - [REQUIRED]
• State License – [REQUIRED]
• Residency Requirements/Fellowship – Completion of an ACGME or AOA approved residency (exceptions may be made by committee) - [REQUIRED]
• DEA required during credentialing, CA fluoroscopy required by dates of coverage so can be submitted without this at time of presentation! - [REQUIRED]
• Malpractice - Clean [HIGHLY PREFERRRED]
• Availability at time of name clear – [REQUIRED]
• A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED] 
• MUST BE WORKING AT A FACILITY AND NOT RIGHT OUT OF RESIDENCY WITH NO ACTIVITY - [REQUIRED]
