- CV Standards: Must include Month & Year for all work history; all gaps must be accounted for and explained.
- Experience: Includes some Chronic Pain Management.
- Patient Mix: Must be comfortable treating a mix of Pediatrics and Adults.

Provider Qualifications
- Education: MD or DO.
- Board Status: Board Eligible (BE)
- Licensure: Active California (CA) Medical License and active DEA (required before start).
Compliance & Immunizations
All documentation must be cleared prior to the start date:
- Flu Vaccine
- TB Status: Results/X-ray within 90 days of the start date.
- Positive Titers or Full Series: MMR, Hep B, and Varicella Zoster.
- Tdap: Must be within the last 10 years.
