Requirements:

Board Certification: Must be BC or BE in Neurosurgery
License Required: active AZ License required, will look at IMLC
DEA: Yes, registered in AZ
CSR: No
Certifications Required: BLS

Minimum Experience Required:

Credentialing Timeframe: 3 days
On average, we have been performing one surgery per day. There are approximately 5-8 consults to round on daily. Expect 4-6 hours per day.
