Description

Hospital is looking for Vascular Surgery APP locum to help with clinic 4 days per week starting ASAP!

Coverage needed: Clinic only 8a-5p, Monday - Thursday. 2-4 weeks per month. Flexible on provider availability

Level II Trauma- HIGH ACUITY and INTENSITY
Client will reimburse for travel
60-90 day credentialing timeframe
Submission Requirements
AANP/NCCPA - Required
ID license or IMLC - Required
DEA/CSR - Required
BLS - Required
Local candidates - highly preferred

Certification Requirements

AANP
Controlled Substance (CSR/CDS)
DEA
NCCPA

State License Requirements
